About the Role
This is a newly created and exciting opportunity for an experienced clinician to step into a pivotal strategic leadership role at Lutheran Homes Group. As our Manager Funding Optimisation & Care Planning, you will lead LHG's Australian National Aged Care Classification (AN-ACC) funding strategy across our SA and Victorian residential care sites  ensuring our funding outcomes truly reflect the quality and complexity of care we deliver every day.

Leading a small dedicated team, you will work closely with clinical teams and Residential General Managers to drive meaningful improvements in care planning, clinical documentation, and funding classification while building internal capability across the organisation. If you are a clinician who thrives at the intersection of clinical excellence and strategic thinking, this is the role for you.

Key responsibilities

• Lead and manage a small team, fostering a culture of clinical excellence and continuous improvement
• Lead LHG's AN-ACC funding optimisation strategy, identifying uplift opportunities and monitoring performance across all homes
• Partner with clinical teams to ensure care planning and documentation practices support optimal funding classification outcomes
• Provide strategic input into admissions decisions to optimise funding mix and financial sustainability
• Develop dashboards and deliver regular reporting and benchmarking insights to senior leaders
• Build capability across clinical and operational teams on AN-ACC principles and documentation excellence
• Ensure ongoing compliance with AN-ACC requirements and keep across regulatory changes

What You Will Need

• Current Registered Nurse registration
• Extensive AN-ACC funding and assessment experience
• Strong understanding of care planning and clinical documentation in residential aged care
• Demonstrated experience in funding optimisation or a similar strategic function
• Strong analytical, problem-solving and stakeholder engagement skills
• Multi-site aged care experience and postgraduate qualifications in health management (desirable)
• Experience leading or mentoring a team (desirable)
• Occasional interstate travel may be required

About the Role

We are currently seeking two Lifestyle Assistants to join our team at our Fullarton Residential Care Facility. In this role, you’ll play an important part in supporting the health and wellbeing of our residents by helping to deliver engaging and meaningful lifestyle programs. Working closely with the Lifestyle Coordinators, you’ll assist in developing and running person-centred activities that encourage wellness, independence, and social connection. You’ll help plan and facilitate individual and group programs based on residents’ interests and abilities, including for those living with dementia.

Key Responsibilities

• Assist with planning and running group and individual lifestyle activities
• Support residents during sessions, including those living with dementia
• Provide 1:1 social and emotional support
• Set up and tidy activity spaces and equipment
• Help with resident mobility and personal care during activities
• Complete basic clerical tasks and record-keeping
• Foster a welcoming, inclusive environment for residents

What You Will Need

• Certificate III in Individual Support (Ageing) or related qualification or a Certificate IV in Leisure and Health
• Previous experience in a similar role, including working in aged care and memory support units with residents living with dementia
• Skilled in planning and facilitating recreational and leisure activities that enhance wellbeing
• Strong communication skills with the ability to build positive relationships with residents, families, and team members
• A person-centred approach with a commitment to dignity, privacy, and respectful care
• Organised, motivated, and reliable, with the ability to work both independently and as part of a team

We are currently seeking an experienced and hands-on Maintenance Team Leader (Retirement Living) to lead the delivery of property maintenance and resident services across our Retirement Living portfolio. This role is responsible for coordinating high-quality, timely and cost-effective maintenance and repair across homes, common areas and grounds, while ensuring services meet legislated and regulatory standards, contractual obligations and resident needs.

You will oversee day-to-day team workloads, provide leadership and support to Maintenance Officers and Gardeners, and help ensure preventative and reactive maintenance programs are delivered safely and efficiently. Working closely with residents, contractors, suppliers and internal stakeholders, you will act as a trusted point of contact for property-related requests, coordinate work orders and contractor performance, and help maintain safe, well-presented and functional environments across the portfolio.

Key responsibilities include:

• Lead day-to-day delivery of property maintenance and resident services across Retirement Living homes, common areas and grounds
• Oversee team workloads, providing direction, guidance and support to Maintenance Officers and Gardeners
• Manage preventative and reactive maintenance schedules, including grounds and seasonal programs
• Coordinate contractors, suppliers, work orders and minor hands-on maintenance to ensure work is completed safely, on time and within budget
• Act as the primary contact for resident maintenance and property-related requests, responding promptly and professionally
• Conduct regular inspections to identify defects, hazards and continuous improvement opportunities
• Ensure compliance with WHS, legislative, contractual and organisational requirements, including safe maintenance of plant, equipment and building systems
• Maintain accurate records, inspection reports, service logs, purchase orders and maintenance documentation in relevant systems
• Provide high-quality customer service by following through on resident enquiries, complaints and urgent issues, and keeping residents informed of progress

• Trade qualifications and/or equivalent practical experience in maintenance, property or facilities services
• Strong experience in facilities or property maintenance, ideally within aged care, retirement living, hospitality or healthcare
• Demonstrated people leadership experience, including team coordination, coaching and performance support
• Strong customer service and communication skills, with the ability to work respectfully with residents, contractors and stakeholders
• Ability to prioritise team workload, respond to urgent issues and balance coordination with hands-on operational support
• Sound understanding of WHS requirements, contractor management and safe maintenance practices
• Strong organisation, problem-solving and record-keeping skills, with confidence using maintenance systems and digital tools
• Confidence using tools and equipment safely, with the physical ability to undertake the practical requirements of the role
• A current Australian driver’s licence and willingness to travel as required; participation in an on-call roster may be required

About the Role

In this role, you’ll provide high-quality cleaning services across our residential sites in Glynde and Hope Valley on a permanent part-time basis, with shifts scheduled Monday to Friday between 8:00am and 4:00pm. As you’ll be required to travel between both locations, a current driver’s licence and reliable vehicle are essential. You’ll help maintain a clean, safe, and welcoming environment by cleaning resident rooms, bathrooms, common areas, and other facility spaces as required.

Key Responsibilities

About the Role

As our Transitional Care Program (TCP) Coordinator, you will be involved in providing assessment, support, care planning, and case management to consumers and their support persons within the community/homebased TCP program. Working closely with consumers, families, allied health professionals, nursing teams and external service providers, to deliver high-quality case management aimed at achieving optimal independence and well-being for consumers.

• Coordinate Transitional Care Program services, ensuring high-quality, person-centred care.

As a Home Care Worker with LHG working across north-eastern Adelaide, you will report to the Client Services Manager and work closely with all Home Care team members. You will provide assistance, support and direct care to consumers in their homes. • Provide all aspects of care to the consumer as per their care plan ensuring that care delivered is of the highest quality

As an Enrolled Nurse in aged care, you will provide compassionate, person-centred nursing care that supports older people to maintain their health, independence, and quality of life. We are currently seeking a Leisure & Lifestyle  Assistant to join our team at our Horsham Residential Care Facility. In this role, you’ll play an important part in supporting the health and wellbeing of our residents by helping to deliver engaging and meaningful lifestyle programs. Working closely with the Lifestyle Coordinator, you’ll assist in developing and running person-centred activities that encourage wellness, independence, and social connection. You’ll help plan and facilitate individual and group programs based on residents’ interests and abilities, including for those living with dementia.

Key Responsibilities

• Assist with planning and running group and individual lifestyle activities
• Support residents during sessions, including those living with dementia
• Provide 1:1 social and emotional support
• Set up and tidy activity spaces and equipment
• Help with resident mobility and personal care during activities

What You Will Need

• Certificate III in Individual Support (Ageing) or related qualification or a Certificate IV in Leisure and Health

• Strong communication skills with the ability to build positive relationships with residents, families, and team members

About the Role

We are seeking an experienced Facilities Coordinator to join LHG in this newly created role, providing leadership and coordination across our Victorian sites in Hamilton, Horsham and Mildura. This is an excellent opportunity for a proactive and hands-on facilities professional to lead an established team while ensuring our sites remain safe, compliant, well-maintained and welcoming for residents, staff and visitors.

This role offers a blend of coordination, stakeholder engagement and operational delivery, with the role being hands-on and field-based, supporting maintenance activities, site inspections and project delivery.

Key Responsibilities

• Lead and coordinate maintenance, grounds and facilities operations across LHG's Victorian sites.
• Ensure compliance with WHS, legislative and preventative maintenance requirements.
• Act as the primary point of contact for facilities and maintenance matters across Victoria.
• Support and oversee capital works and Individual living unit refurbishment projects.
• Coordinate contractors, suppliers and work orders to ensure services are delivered safely on time and within budget.
• Manage preventative, reactive and grounds maintenance programs to maintain high site standards.
• Partner with Site General Managers to align facilities priorities with operational needs.
• Maintain accurate records, reporting, purchase orders and maintenance system updates.
• Deliver exceptional customer service and communication to residents, staff and stakeholders.

What You Will Need

• Strong stakeholder engagement is essential, working collaboratively with residents, site leaders and operational teams.
• Trade qualifications and/or equivalent experience in facilities, maintenance or property management preferred
• Experience within Facilities Management, Aged Care, Retirement Living or Hospitality industries - preferred
• Experience coordinating maintenance, grounds or capital works activities across multiple sites
• Demonstrated ability to manage competing priorities and coordinate works across a geographically dispersed portfolio
• National Police Clearance and Driver’s license

This role involves overnight travel across the region, with accommodation provided as required.

Access to a pool vehicle will be provided for work-related travel.
