About the Role

We are currently seeking a dedicated Registered Nurse to provide high-quality, person-centred clinical care to older people living in the community. The role combines clinical assessment, care planning and coordination with close collaboration across a multidisciplinary team, families and external providers. You will monitor changes in consumers’ health and wellbeing, make sound clinical decisions within scope of practice, and support safe and effective care delivery across a range of complex and routine needs. The position also contributes to quality improvement, incident management and the supervision and support of care staff, ensuring services align with the Aged Care Quality Standards and organisational policies.

 

Key responsibilities include:

  • Conduct initial and ongoing clinical assessments and develop, review and update individualised care and service plans.
  • Work collaboratively with consumers, families and health professionals to deliver safe, person-centred care aligned to goals and preferences.
  • Monitor changes in condition or behaviour and respond appropriately, including escalation to relevant clinicians when required.
  • Provide and oversee clinical care including medication management, wound and skin care, continence support, nutrition, palliative care and chronic disease management.
  • Respond to acute clinical incidents (e.g. falls, choking, deterioration) and report to the Care Manager.
  • Liaise with hospitals, discharge planners and external providers to coordinate appropriate services following admission, discharge or respite.
  • Supervise and support care staff, provide guidance on best practice and address performance or practice concerns promptly.
  • Ensure documentation is accurate, timely and compliant with regulatory and organisational requirements.
  • Manage and report incidents in line with the Serious Incident Response Scheme (SIRS) and infection control protocols.
  • Participate in audits, quality improvement activities and collection of quality indicator data.
  • Promote safe work practices and contribute to a culture of continuous improvement and respectful care.

 

About You

  • AHPRA Registration as RN with experience in aged care or subacute care
  • Demonstrated clinical assessment and care planning capability
  • Strong communication skills with ability to build rapport with older people and families
  • Ability to handle sensitive matters with professionalism and confidentiality
  • Accurate documentation and attention to detail
  • Proficiency using clinical systems and electronic health records

Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.
