About the RoleThe Care Manager plays a key role in ensuring each consumer receives high-quality, person-centred care that respects their strengths, choices, and individuality. Through a capabilities-based approach to admission, assessment, care planning, delivery, and ongoing evaluation, the Care Manager supports people to live well and with dignity. Reporting to the General Manager Residential Care, this role works closely with Care Coordinators, clinical governance, multidisciplinary teams, and health professionals to create a supportive, collaborative environment where consumers are empowered to live their best life, their way each and every day. Key responsibilities include: - In collaboration with the General Manager, Care Coordinators and Quality and Education Coordinators, use evidence-based practice in developing and managing a Program of Continuous Improvement to deliver quality care to consumers.
- Responsible for all aspects of care under the direction of the General Manager Residential Care and ensuring best practice and compliance with the Aged Care Quality Standards and Code of Conduct for Aged Care
- Ensure high quality, holistic, person-centred care and services to consumers, in accordance with ANMCI Competency Standards and consumer choices and preferences
- Ensure the reporting of incidents in accordance with Serious Incident Response Scheme (SIRS) and NDIS requirements.
- Work closely with the General Manager in workforce planning and assist with roster management as required
- Assist in the development of clinical staff through on-going coaching and feedback
- May be required to cover General Manager Residential Care absences and to participate in an out-of-hours on call manager roster.
About You- Previous experience as a Care Coordinator, Clinical Nurse, or Care Manager in aged care, or as an Associate Nurse Unit Manager (ANUM) or Nurse Unit Manager (NUM) in a relevant acute care setting.
- Registered Nurse with AHPRA registration
- This role is considered a responsible person under the Aged Care Act and required to meet the associated suitability matters.
- Strong clinical skills
- Ability to deal with sensitive matters with tact, discretion and appropriate confidentiality
- Passion for and commitment to aged care
