About the Role

Join our team in an exciting role as an Allied Health Professional - Physiotherapist, where you will make a difference to our Transitional Care Program (TCP) clients to help them regain their independence and optimise quality of life. The TCP program provides rehabilitation and engagement for older people through person-centred model of care which can include individualised, group or home-based program.

 

As part of our dynamic Allied Health team, you will help shape the direction of Allied Health services within our organisation. Collaborate closely with our supportive team, including the Allied Health Team Leader, other Allied Health Professionals, Site Care Manager, and Clinical team members in both Residential, Community and Rehabilitation. You will work alongside other health team members to understand client care needs, plan interventions, and review outcomes, supporting residents and clients to achieve their goals. Key Responsibilities

  • Provide clinical support to the Residential, Transitional Care and Community program teams and act as an escalation point
  • Provide oversight of external provider’s clinical assessments and equipment prescription
  • Facilitate group programs as directed by treating therapist and support Leisure and Lifestyle program on site.

About You:

  • Relevant qualification in an Allied Health profession (Physiotherapy) with demonstrated experience in area of expertise
  • Current registration with relevant registration authority as per legislation
  • Ability to provide education, support and coaching to direct care employees, clients and their families
  • Well-developed communication (written and face-to-face) and interpersonal skills to liaise with clients, carers families and direct care employees
  • Knowledge of the aged care quality standards (desirable)
  • Leadership experience in residential aged care or home care (desirable)

Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.
