About the RoleIn this role, you will assess and develop individualised reablement and rehabilitation programs, working closely with families and healthcare professionals to enhance the quality of life for our clients. By supporting community clients across various locations, you’ll help them regain independence and improve their overall well-being. As a key member of our dynamic Allied Health team, you’ll collaborate with the Allied Health Team Leader, other professionals and Clinical staff to shape the future of our services. You will assess client needs, plan interventions, and review outcomes to ensure clients achieve their goals. Key Responsibilities will include: - Conduct a thorough physiotherapy assessment, develop and deliver goals focused interventions.
- Provide clinical support to the community program team and act as an escalation point for complex cases.
- Collaborate with multidisciplinary teams to ensure comprehensive client care and improved outcomes.
- Ensure physiotherapy clinical assessments meet quality standards and align with client needs.
- Provide equipment prescription to ensure appropriate, safe, and effective solutions for clients.
- Provide support to Team Leader in all aspect of community program delivery
- Deliver health promotion activities for LHG communities
About You:- Relevant qualification in an Allied Health profession (Physiotherapy) with demonstrated experience in area of expertise
- Current registration AHPRA
- Ability to provide education, support and coaching to direct care employees, clients and their families
- Well-developed communication (written and face-to-face) and interpersonal skills to liaise with clients, carers families and direct care employees
- Knowledge of the aged care quality standards
- Leadership experience in residential aged care or community care (desirable)
Candidates will be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance.
