About the RoleAs a Home Care Enrolled Nurse with Lutheran Homes Group, you will work in collaboration with the Care Manager, Home Care and Registered Nurses to provide consumers with quality person-centred care. Being a passionate advocate for our consumers, you will support and guide care staff to deliver best practice care services without compromising consumers personal preferences and right to choice. Key responsibilities include: - Provide direct care to consumers, in consultation with the rest of your team, in order to achieve optimal care outcomes for the consumer with consideration for their individual needs, preferences, choices and culture.
- Interpret information from a range of sources in order to contribute to planning, developing, monitoring and maintaining appropriate care.
- Recognise and escalate changes in consumers’ health and well-being and take appropriate action including liaising with the Care Manager, consumer’s medical practitioner(s) and, where appropriate, next of kin
- Ensure safe and appropriate medication administration, within the scope of your role, in consultation with the Registered Nurse, and where necessary, with the pharmacist and/or medical practitioner.
- Collaborate with multidisciplinary team members, to ensure quality services are provided to consumers in accordance with consumer’s preferences and choices.
- Provide guidance to Home Care Workers as required to ensure they understand their responsibilities and perform to the best of their abilities.
About You- Diploma in Nursing Qualification and registered as an Enrolled Nurse with AHPRA
- Strong organisational, time management and prioritisation skills
- Minimum 2 years community experience
- Ability to deal with sensitive matters with tact, discretion and appropriate confidentiality
