About the Role

The Community Transitional Care Program (TCP) Coordinator – Occupational Therapy or Physiotherapy Lead is primarily responsible for coordinating and delivering person-centred, goal oriented transitional care program that supports clients transitioning from hospital or Residential Transitional Care settings to home settings.

 

The CTCP Coordinator will draw on advanced Occupational Therapy or Physiotherapy expertise to provide clinical leadership and establish collaborative relationships with clinical teams, care services, and other agencies to deliver high-quality case management aimed at achieving optimal functional independence, safety and well-being of the clients. This role involves providing assessment, support, care planning, and case management to consumers and their support persons within the community/home-based TCP program.

 

Key responsibilities include:

  • Develop and implement individualised goal focused reablement care plans that address functional goals, barriers and support needs
  • Facilitate complex case discussions and support clinical decision-making across the allied health and nursing teams.
  • Act as the subject matter expert on safe discharge planning, home safety assessments, and adaptive equipment recommendations
  • Ensure the TCP program is delivered to a high standard through developing, maintaining and applying strong technical skills.
  • Lead service review, evaluate, and quality improvement initiatives related to functional outcomes, safety and client satisfaction
  • Lead the multidisciplinary team in clinical planning, implementation, and evaluation
  • Provide advanced physiotherapy or Occupational Therapy assessments and interventions when required

 

About You

  • This role is considered a NDIS risk assessed role and a current NDIS Worker Clearance is required.
  • A current Australian driver’s licence (P2 or above) and a willingness to drive is preferable.
  • Demonstrated clinical expertise in Occupational Therapy or Physiotherapy practice within community, sub-acute, or aged care settings.
  • Demonstrated practise/experience with at least 3 years, full time equivalent, post registration experience.
  • Tertiary qualifications in Occupational Therapy or Physiotherapy.
  • Current registration with (AHPRA)

Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance. In accordance with current health guidelines for the aged care sector, candidates are encouraged to have all recommended vaccinations.
